BACKGROUND: The prevention of dental caries has always remained a challenge. Caries prevention through dietary intervention holds promise. Studies have revealed that several constituents present in tea have anticariogenic properties. Tea is a widely consumed beverage and hence could be utilized as a suitable caries preventive agent. The purpose of this study was to determine the effect of black tea on caries progression in experimental animals. MATERIALS AND METHODS: This study was carried out in 17-day-old albino rat pups. The animals were divided into three groups, with eight animals in each group. They were fed on a cariogenic diet (MIT 200) and inoculated with Streptococcus mutans. Group I was given MIT 200 with water, Group II was placed on MIT 200 with black tea, and Group III was placed on MIT 200 with fluoridated water for a period of 45 days. After 45 days, the animals were killed under ether anesthesia, and their teeth were examined for caries. RESULTS: The carious lesions were scored for the first two molars in each quadrant. In each group, a total of 64 teeth were examined. The caries score between the upper and lower jaws was compared using ANOVA. CONCLUSION: From this study, it may be inferred that drinking black tea reduced the development of dental caries in young rats fed on a cariogenic diet. The tea used for this study was prepared using fluoride-free water, so we can assume that besides fluoride, certain components are present in tea leaves that possess anticariogenic properties.

BACKGROUND: Enterococcus faecalis is the most persistent organism in the root canal which resists most of the intracanal medicaments. There is always a constant attempt to eliminate this endodontic pathogen from the root canal system. AIM: The aim of this study was to evaluate the efficacy of the association of different concentrations of proton-pump inhibitor (PPI) (Lansoprazole) with calcium hydroxide (CH) and chlorhexidine (CHX) against E. faecalis using a broth dilution method. MATERIALS AND METHODS: E. faecalis was inoculated into brain-heart infusion broth at 37°C for 5 h. The master broth was then treated with CH (Group 1); CH + 2% CHX (Group 2); CH + PPI 6.25 µg/ml (Group 3A); CH + PPI 25 µg/ml (Group 3B); 2% CHX + PPI 6.25 µg/ml (Group 4A); 2% CHX + PPI 25 µg/ml (Group 4B); CH + 2% CHX + PPI 6.25 µg/ml (Group 5A), and CH + 2% CHX + PPI 25 µg/ml (Group 5B). The groups were spectrophotometrically analyzed at 630 nm at 24 h to determine the group with the least optical density. STATISTICAL ANALYSIS: Comparison between the groups was done by the one-way analysis of variance and Kruskal-Wallis test for multiple comparisons. RESULTS: The mean percentage inhibition of E. faecalis by Group 5A (CH + 2% CHX + PPI 6.25 µg/ml) was the highest compared to other groups. The lowest mean value was observed in Group 3A (CH + PPI 6.25 µg/ml) indicating least efficiency. CONCLUSION: There was a concentration-dependent effect of PPI on CH and CHX against E. faecalis. The maximum efficacy was found when the lower concentration of PPI was associated with CH/CHX mixture.

BACKGROUND: Apical periodontitis (AP) is one of the most prevalent diseases of the teeth. Treatment of AP is based on the removal of the cause, i.e., bacteria from the root canals. Achievement of adequate bacterial eradication in one appointment treatment remains a controversy. AIM: This prospective study was conducted with the objective to compare the periapical healing of teeth with AP treated in (a) single visit versus (b) two visits, either with or without Vitapex as an intracanal medicament. SUBJECTS AND METHODS: Patients were selected randomly from the Department of Conservative Dentistry and Endodontics. Forty-three patients (81 teeth) met the inclusion criteria, i.e., AP (both symptomatic and asymptomatic) visible radiographically size ≥2 mm × 2 mm, not suffering from any immune-compromising disease, age between 16 and 65 years and tooth not accessed previously. Patients were randomly divided into three groups, i.e., single-visit group (Group 1), multi-visit group without any intracanal medicament (Group 2), and multi-visit group with Vitapex as interim intracanal medicament (Group 3). Comparison was done radiographically using periapical index (PAI). The primary outcome measure was the change in periapical radiolucency after 1 year assessed by PAI scores. The Mann-Whitney U-test was used to evaluate differences between groups at baseline (immediate postoperative) and at the 12-month follow-up evaluation. Change in PAI score for each group from baseline to 12-month follow-up evaluation was tested with the Wilcoxon signed rank test. The secondary outcome measures, proportion of teeth in each group that could be considered improved (decreased PAI score) or healed (PAI <2), were assessed with the Chi-square test. RESULTS: No statistically significant difference in periapical healing was found between three groups. CONCLUSION: After 1-year evaluation, no difference in periapical healing was found between single-visit treatment and multi-visit treatment groups with the given sample size.

AIM: To evaluate the effectiveness of different techniques in removing calcium hydroxide (Ca(OH)2) from the root canal. MATERIALS AND METHODS: Twenty-four freshly extracted mandibular premolars were instrumented using ProTaper rotary instruments. The teeth were longitudinally split into two halves, cleaned of debris. The two halves were then reassembled and filled with Ca(OH)2 and were divided into four groups. In Group I, the teeth were irrigated with 5 mL of 2.5% sodium hypochlorite (NaOCl) and 5 mL of 17% of ethylenediaminetetraacetic acid. In Group II, the teeth were irrigated with 5 mL of 2.5% NaOCl and a rotary ProTaper F3 instrument was used. In Group III, the teeth were irrigated with 5 mL of 2.5% NaOCl and agitated using an ultrasonic unit. In Group IV, the teeth were irrigated with 5 mL of 2.5% NaOCl and a CanalBrush was used to remove Ca(OH)2. The roots were disassembled, and photographs were taken. The amount of residual Ca(OH)2 was calculated using an image analysis software as a percentage of the total canal surface area. The data were analyzed using one-way analysis of variance and post-hoc Tukey test. RESULTS: CanalBrush and ultrasonic techniques showed significantly less residual Ca(OH)2 than irrigants and rotary techniques. There was no significant difference between the rotary and irrigant techniques. CONCLUSION: None of the techniques used were completely able to remove Ca(OH)2 from the root canals. But the CanalBrush and ultrasonic techniques were significantly better than the rotary instrument and irrigant groups.

BACKGROUND AND OBJECTIVES: The present study was undertaken to compare and evaluate the area increase of root canals with ProTaper, iRaCe and Revo-S systems using cone beam computed tomography for analysis. MATERIALS AND METHODOLOGY: Forty five extracted human mandibular premolars having single canal and straight root were collected. Teeth were randomly assigned to three groups (n=15). Samples were decoronized by maintaining root length at 14 mm. Pre-instrumentation cone beam computed tomography scan was done after stabilizing the samples on wax blocks. The working length was determined at 1 mm short from the apical foramen by using a ISO 15 K-file tip protruding at apical foramen. Preparation was carried out according to the manufacturer's instructions. Finally, canals were instrumented upto 30/.06 apically for each group. After each instrumentation, root canals were irrigated with 2ml of 3% sodium hypochlorite solution followed by 2 ml of 17% EDTA solution. Final irrigation was done with 5ml of saline. Post instrumentation cone beam computed tomography scans of all samples in the 3 groups were acquired. RESULTS: Mean percentage of area increase in different thirds of the canal was highest for ProTaper followed by i-RaCe and Revo-s system which was statistically significant. INTERPRETATION AND CONCLUSION: Root canal area increase was highest for ProTaper followed by i-Race and Revo-S systems.
